About the role: 

We have an exciting opportunity for a full time Specialist Support Coordinator to join our Darwin team.

The position will be intensive and time sensitive case management for participants of the NDIS who have complex needs and require additional oversight. You will identify and address barriers to ensure successful implementation of their NDIS plans.

You will build relationships with participants, their families, and support networks, to deliver person-centred supports to achieve positive and successful outcomes.

You will be liaising with various government agencies including: National Disability and Insurance Agency, Quality & Safeguards Commission, Department of Health, Office of the Public Guardian and Trustee, Justice, and Education.

You will develop a collaborative working relationship with key stakeholders and disability service organisations.

 

Key Responsibilities:

  • Develop and maintain effective, collaborative, and professional relationships and networking with all stakeholders.
  • Develop complex service plans to address barriers.
  • Crisis management: planning and prevention.
  • Coordinate significant life transitions.
  • Facilitate regular stakeholder meetings.
  • Complete clear and accurate reports and case notes as per NDIA and organisational policy.
  • Promote and maintain a positive, respectful, and enthusiastic work environment.

 

Key Requirements:

  • A bachelor’s degree in social work, nursing, or other allied health discipline.
  • Highly developed written communication skills with the ability to maintain strong attention to detail.
  • Highly developed interpersonal and communication skills.
  • Demonstrated ability to work independently and cooperatively as part of a team.
  • Well-developed organisational skills and the ability to prioritise and complete work within established deadlines.
  • Established critical thinking and problem-solving skills.
  • Computer literacy.

 Desirable Criteria

  1. Previous experience in the not for profit sector, or a demonstrated appreciation and understanding of community-based organisations.

 

 Employment Requirements

  1. Current Northern Territory Drivers License 
  2. Current National Criminal History Check 
  3. Working with Children Clearance (NT Ochre Card)
  4. NDIS Worker Screening and Orientation
  5. First Aid and CPR Certificate
  6. Vaccinations – COVID-19 with Booster, Influenza
