About the role 

The Clinical Care Coordinator provides clinical leadership across all aspects of the care operations of the organisation. Working collaboratively with all levels of management, participants, families and guardians, disability support workers, and relevant community agencies to facilitate high quality, effective and participant focused care.

 

What you can bring

  • Degree in nursing or allied health and significant experience in a similar role in a community, disability or aged care setting.
  • Current registration with appropriate professional body, if applicable (AHPRA etc)
  • Extensive and demonstrated understanding of the National Disability Insurance Scheme (NDIS), the NDIS Quality and Safeguarding Framework, current legislative requirements, national and international conventions related to people with disability and the implications of these frameworks/principles
  • Well-developed clinical judgement skills with the ability to apply critical thinking and problem-solving skills to make sound decisions.
  • Highly developed interpersonal and communication skills, including ability to undertake complex negotiations and develop networks, relationships and partnerships with internal and external stakeholders.
  • Highly developed written communications skills with the ability to maintain strong attention to detail.
  • High level of professionalism including the confidential management of all organisational, participant and employee information.
  • Ability to implement change in and improvements in care practices
  • Ability to prioritise, organise and complete work within established deadlines
  • Demonstrated understanding of issues affecting people with disability from culturally and linguistically diverse backgrounds and communities.
  • Demonstrated staff supervision skills

 

Highly desirable

  • Previous experience in the not-for-profit sector, or a demonstrated appreciation and understanding of community-based organisations.
  • Experience in conducting competency assessments for care workers.
