About the role

We are seeking a motivated, collaborative, and person-centred individual to join our team as a Support Coordinator.

Based in Palmerston, this vital role helps NDIS participants understand, implement, and manage their NDIS plans. You’ll empower participants to exercise choice and control, connect with the right services, and build the skills and confidence to manage their supports independently over time.

What you’ll be doing

As a Support Coordinator, you will:

  • Provide individualised support to help participants understand and action their NDIS plans
  • Collaborate with participants, families, and providers to develop person-centred plans and solutions
  • Identify innovative services and community supports aligned with participant needs
  • Facilitate referrals, service agreements, and ensure smooth coordination of services
  • Support participants to exercise choice and control, make informed decisions, and achieve their goals
  • Monitor service delivery, manage risks, and resolve barriers impacting participants’ progress
  • Prepare for NDIS plan reviews, including compiling reports and supporting documentation
  • Maintain accurate records and uphold high standards of confidentiality and compliance

 

What you’ll bring

You’ll bring strong relationship-building skills, problem-solving ability, and a genuine passion for empowering people with disability.

We’re looking for someone with:

  • Experience working in disability, community services, social work, or similar human services fields
  • Knowledge of the NDIS, or the ability to quickly acquire this knowledge
  • Strong understanding of choice and control principles and participants’ rights
  • Excellent verbal and written communication skills
  • Ability to problem-solve, negotiate solutions, and manage competing priorities
  • High standards of confidentiality and professional integrity
  • Proficiency in digital tools and record-keeping systems

 

You will also need to hold (or have the ability to obtain):

  • A current Ochre Card
  • National Criminal History Check
  • NDIS Worker Screening Clearance
  • First Aid and CPR certificate
  • COVID-19 and Influenza vaccinations
  • Australian work rights
  • NT Driver Licence
