About Somerville Community Services

Somerville is a for-purpose, community-based organisation that has supported Territorians for over 50 years. We specialise in high-quality disability services that empower people to live the life they choose. As a registered NDIS provider, we deliver individualised support across the Northern Territory, with a focus on respectful, person-centred relationships that promote choice, dignity, and independence.

About the role

We’re seeking a skilled and compassionate Specialist Support Coordinator to join our Support Coordination Services team in Alice Springs.

The Specialist Support Coordinator supports NDIS participants with complex needs, high risks, or involvement across different service systems. You’ll work closely with participants, families, and providers to reduce barriers, stabilise supports, and ensure plans are implemented safely and effectively.

This role combines case coordination, problem-solving, and advocacy to support participants to achieve meaningful outcomes while navigating complex circumstances.

What you’ll be doing

As a Specialist Support Coordinator, you will:

  • Provide specialist-level support coordination to participants with complex needs and risks.
  • Collaborate with participants, families, carers, and service systems such as health, housing, justice, and safeguarding agencies to develop and implement practical solutions.
  • Respond to changes in circumstances and crises, ensuring timely coordination of safe and effective supports.
  • Support participants through significant life transitions and prepare detailed reports and evidence for NDIS plan reviews.
  • Advocate for participants’ rights and preferences while balancing safeguarding responsibilities and risk management.
  • Maintain accurate case notes, records, and reports in line with NDIS Practice Standards and organisational policies.

What you’ll bring

You’ll bring strong relationship-building skills, advanced problem-solving ability, and a genuine commitment to supporting participants with complex needs.

We’re looking for someone with:

  • Experience working in disability, community services, social work, or similar human services fields, including supporting participants with complex needs
  • Knowledge of the NDIS and Support Coordination, or the ability to quickly develop this expertise
  • A strong understanding of participant rights, choice and control, and how to balance these with safeguarding responsibilities and risk management
  • Excellent verbal and written communication skills, with the ability to prepare clear reports and build trusted relationships with diverse stakeholders
  • The ability to problem-solve, negotiate solutions, and overcome barriers to services
  • Strong organisational skills to manage competing priorities and meet required outcomes
  • High standards of confidentiality, professional integrity, and culturally safe practice
  • Proficiency in Microsoft Office and client management systems for accurate record-keeping and reporting

You will also need (or have the ability to obtain):

  • NT Driver’s Licence
  • National Criminal History Check
  • NT Ochre Card (Working with Children Clearance)
  • NDIS Worker Screening Clearance
  • COVID-19 and Influenza vaccinations
  • First Aid and CPR Certificate
  • Full Australian working rights

Please refer to the full position description for a complete list of essential and desirable criteria.
