Who are we?

Wintringham provides aged care services to those who are financially disadvantaged and have previously been homeless or are at risk of becoming homeless. We employ over 850 staff and our team of Carers, Case Managers and Outreach Workers provide holistic care and support to clients who live independently in their own homes.

 

Your new role

Wintringham is seeking a flexible and proactive Client Services Coordinator to join our friendly and tight-knit team at our Keysborough office to work across our community programs based in South Eastern Melbourne & Morning Peninsula regions. Reporting to the Regional Manager Community Services, you will be developing the profile of Wintringham with external agencies and networks, as well as supporting our care and support team. This is a part time role for two days per week working 5 hours each day.

 

Your duties and responsibilities will include:

  • Following up enquiries, coordinating and conducting sign-ups for homecare packages
  • Developing new and building on existing partnerships with key agencies.
  • Providing supervision to ensure that care and support workers receive monthly supervision, and that any matters of concern regarding their work performance are addressed in a proper and time effective manner and that they participate in an annual performance appraisal.
  • Ensuring Case Managers meet administrative requirements of their roles
  • Practicing open communication and pro-actively participate in problem solving where issues or areas of disagreement arise, as per the Open Communication Policy found in the Leadership and Management Manual.
  • Maintaining information in the SamCare, GoldCare and My Aged Care.

What we will require from you:

  • An approved tertiary qualification in Social Work, Nursing, Allied Health or Welfare.
  • Experience in Case Management
  • Experience in the Community Aged Care setting
  • Knowledge of GoldCare software (highly advantageous)
  • Knowledge and understanding of the Aged Care standards
  • Current drivers licence with a reliable vehicle suitable for transporting clients
  • Knowledge of housing and support issues facing older homeless people whom may have a physical and/or mental disability
  • Excellent computer skills in particular, knowledge and understanding of Microsoft Excel
