We are seeking a Team Facilitator for a talented, creative gentleman’s support team based in Caloundra. The Team Facilitator will play a pivotal role, ensuring that day-to-day responsibilities are met while supporting the gentleman to live a safe, healthy and comfortable lifestyle in his own home. Key responsibilities include developing personalised support strategies, managing staff, and ensuring that all support services are delivered in alignment with his aspirations and the family's values.
Key Responsibilities:
- Support the gentleman’s vision for a unique lifestyle by implementing creative and effective support strategies.
- Collaborate with family members to ensure that his home reflects his preferences and values.
- Manage the scheduling and rostering of support staff within budgetary constraints.
- Develop and maintain strong relationships with the family, understanding their needs and providing support as required.
- Assist in documentation to communicate the gentleman’s skills, interests, and needs to all support staff.
- Facilitate training and development for support workers, ensuring they provide high-quality care.
- Oversee the recruitment, induction, and performance management of support staff in line with organizational policies.
- Implement safeguards to ensure dignity, privacy, and well-being are always upheld.
- Coordinate communication between support staff and the family, ensuring that all parties are informed of any changes or updates.
Candidate Profile:
The ideal candidate will have a strong commitment to person-centered care and a passion for supporting individuals with diverse needs. You will possess excellent leadership and communication skills, as well as the ability to work collaboratively with families and support teams.
- Proven experience in a leadership role within a support or care setting.
- Strong understanding of person-centered practices and the importance of community inclusion.
- Exceptional organisational skills with the ability to manage multiple priorities.
- Empathy and understanding towards individuals with unique needs and their families.
- Aa proactive leader who thrives in a collaborative environment and is dedicated to making a positive impact in the lives of others.
Shifts/Hours:
- 3 – 4 days per week working in the team as support worker lead
- Flexibility to pick up additional shifts when required
Essential requirements:
- +5 years experience Disability support work
- +3 years experience in a Team Leader / Key Worker Role or equivalent position
- Non-Smoker
- Open Driver’s license and own vehicle with insurance
- Live within reasonable travelling distance
- Current First Aid Certificate & CPR
- NDIS Worker Screening/Yellow Card (or willing to obtain)
- NDIS Worker Orientation Module Certificate (or willing to obtain)
- Blue Card (or willing to obtain)
- Flexibility to pick up additional hours when needed
