In partnership with other providers, our HomeConnect program provides an operation of prevention and early intervention services for people who have experienced, or are at risk of, homelessness
A little bit about the role
We are seeking a compassionate and dedicated Key Worker to join our HomeConnect program. As a Key Worker, you will work within a transdisciplinary team to provide person-centered case management to individuals and families at risk of homelessness. Your role will involve supporting clients to secure and maintain stable housing, linking them with essential services, and advocating on their behalf.
Responsibilities:
- Collaborate with the HomeConnect team to help clients secure and sustain rental accommodation in both private and community housing sectors.
- Assist clients in connecting with community resources, including financial counselling, education, training, and employment services.
- Advocate for clients to ensure equitable access to services.
- Develop and implement tailored support and housing plans, guiding clients through their transition to stable housing.
- Engage with clients using a strength-based, person-cantered approach that respects diversity, equality, and client participation.
- Address complex support needs, including those related to trauma, mental health, and family violence.
- Maintain accurate and timely client records in line with relevant legislation.
What We're Looking For:
Qualifications:
- Relevant qualifications in social work, community development, or a related discipline.
Experience & Skills:
- Experience in the homelessness, housing, or support services sector.
- Proven ability in providing case management to marginalized clients.
- Strong understanding of the challenges facing individuals experiencing homelessness, including mental health, AOD (Alcohol and Other Drugs), disability, and trauma.
- Demonstrated experience working across various service systems, such as mental health, public health, family violence, or justice systems.
- Current and valid Working with Children’s Check and full Victorian Driver’s Licence.
