Little bit about the role 

The Senior Young Adults Outreach Case Manager (Aboriginal & Torres Strait Islander Portfolio) plays a vital role within VincentCare’s Young Adults Outreach Support Service (YAOSS), based at our Northern Community Hub. The program supports young people aged 17–25 who are homeless or at risk of homelessness, many of whom face challenges including mental health, substance use, disability, justice involvement and family violence.

In this role, you will provide intensive case management, working alongside young people to secure safe and sustainable housing and to build pathways toward greater stability and independence. You will also hold a dedicated portfolio providing specialist support to Aboriginal and Torres Strait Islander young adults, building strong connections with community leaders, services and networks to ensure culturally safe, respectful and responsive support.

This senior position provides leadership in case management practice, guidance to colleagues, and contributes to service development and continuous improvement across the program. Key Selection Criteria

  • Relevant tertiary qualifications and/or direct experience in homelessness, housing and support services sector

  • Experience supporting young people at risk of or experiencing homelessness, with sound knowledge of the housing and related service systems.

  • Proven ability to deliver trauma-informed, holistic support across complex needs (mental health, AOD, disability, justice, family violence).

  • Experience working with and supporting Aboriginal and Torres Strait Islander young people, with strong cultural awareness and community engagement skills.

  • Experience mentoring colleagues and contributing to program or service development.

  • Excellent communication and relationship-building skills with strong professional boundaries.

  • Ability to work both collaboratively and independently.

  • Strong organisational, time management, and problem-solving skills.

  • Resilience, sound judgement, and capacity to manage risk in complex situations.
