We are currently looking for a Community Care Coordinator to join our team supporting clients across the Sydney area

About the role:

As a Community Care Coordinator, you will partner with clients and assist them to remain independent at home and in their community.   You will coordinate the delivery of care and services to clients, leading a team of care workers to provide high quality services.

  • Previous experience in a similar position.
  • High level of written and oral communication skills
  • Previous experience of working with staff and clients from a CALD background
  • Ability to use various information technology systems
  • Demonstrated skill in coordinating a flexible, multi-skilled team working environment

Desirable

  • Demonstrated knowledge of the Aged Care sectors including funding protocols and legislative standards and compliance requirements.
