Your new role
Wintringham is seeking a dynamic and proactive Case Manager, to join our South Tasmanian multi-disciplinary community team on a short term, full time basis. This dynamic full time opportunity will work with our rostering team to make sure that clients receive the support and care needed based on their funded level of package, 5 days a week, Mondays to Fridays. This role has a short term focus on supporting clients and staff as we work to transition Home Support services out of Southern Tasmania.
Your duties and responsibilities will include:
- Assisting clients living in their own homes to maintain their independence and lifestyle of their choice by providing a high level of care.
- Work with Home Support Management and Projects Team to close the services in Tasmania under Home Support (CHSP and Support at Home).
- Support current clients to transition across to alternate providers including Anglicare Tasmania
We are looking for candidates with the following:
- An approved tertiary qualification in Social Work, Allied Health or Welfare.
- Previous experience as a Case Manager (would be highly regarded)
- High level computer, documentation, and budgeting skills (mandatory)
- Current drivers’ licence
