Who are we?

Wintringham is a not-for-profit organisation providing an extensive range of aged care, housing and related support services to men and women aged 50-plus who have been homeless or at risk of homelessness.  We employ over 800 staff and operate from multiple offices across Melbourne and regional Victoria.


We are very excited to announce the expansion of our services into Tasmania and are looking for a passionate and motivated individual to be a part of our new venture. The launch of our community services is the first phase of establishing specialised services for vulnerable older Tasmanians, focusing on secure, stable housing with close and accessible supports at the ready.


Your new role

We are currently seeking proactive individuals to join our Tasmanian Team as a Pre Home Care Package Case Manager. Reporting to the Housing & Homelessness Support Coordinator (Tasmania), you will work closely with the Aged Care Assessment Services to provide support and Case Management while vulnerable clients are awaiting an assignment of a Commonwealth Home Care Package.  You will provide assistance to clients to help them, obtain access to additional health, welfare and home care services through referrals and case management. You will also assist clients to navigate their offer of Home Care, when available, to ensure they select a suitable provider, and do not miss their Home Care Package allocation.


Your responsibilities and duties will also include:

  • Locating and establishing rapport with homeless and socially isolated people as well as people with challenging behaviours
  • Providing immediate, flexible and practical assistance to help overcome pressing needs or crises experienced by the client.
  • Consulting and providing information about the range of services to targeted diverse communities and the individuals within them
  • Providing short-term, episodic support to clients who need services at key stages of their care pathway
  • Use strategies to empower and build the confidence of clients and their carers to access and use services
  • Assessment of clients, taking into account their history, circumstances and preferences.

We are looking for candidates with the following:

  • A tertiary qualification in Social Work, Allied Health or relevant qualification
  • Experience in case management or case coordination with older people
  • Experience in working effectively and sensitively with a broad range of clients, including those with high support needs and challenging behaviours
  • An understanding of the Housing and Homelessness Service sector My Aged Care, NDIS and other relevant service systems.
  • Proven commitment to excellence, accountability and innovation in best practice service delivery in the fields of homelessness and housing support.
  • Well-developed communications skills and the ability to with engage people from varying backgrounds.
  • Current drivers licence
