Support Coordinator

At The Disability Trust we understand how important it is to find the best options to meet our client's needs. Our Trusted Support Coordination team are skilled at working with clients to help find and choose providers, linking the best value and quality services available. We are looking for a local who knows the Illawarra/Nowra region and is highly skilled at sourcing a diverse range of supports, from specialist disability to therapeutic services and mainstream supports for a 12-month fixed term contract. 

About the role
As a Support Coordinator, you will be responsible for working with participants and their families to get the most out of their NDIS plan. You will assist participants to strengthen their ability to manage their own plan and in turn, enable greater independence across all domains of their life. Time management is paramount, as you ensure administrative responsibilities are met, as well as planning ahead of time to prepare for plan reviews. 

As the intermediary for service establishment, you will be a positive and influential representative for your client. This role includes researching, coordinating and seeking support for participants. You will also be responsible for establishing relationships necessary to support your client, to increase inclusivity within their community. Choice and control must be forefront while assisting to choose and evaluate supports that will assist participants in meeting their goals. The successful candidate has the option to be located at either our Nowra or Shellharbour office, with travel to the other office as needed. Flexible working from home arrangements are also available, however you will be required to travel across the Illawarra region regularly.  

About you
You will maintain a highly customer-centric approach and professionalism as you assist your client to determine and secure appropriate services. Ideally you will also have:

  • An understanding of the NDIS and its impact on the lives of people with a disability
  • Advanced communication skills, which will aid in navigating a range of activity and functions within the community
  • An ethical and professional approach to work to maintain the dignity and privacy of participants
  • The ability to work independently and with limited supervision in your day-to-day work
  • Excellent time management skills to ensure administrative responsibilities are met
  • Familiarity with local services and opportunities within the community

A relevant qualification (Disability, Health, Psychology, social work or education) would be desirable but not essential. As well as experience working with people with a disability in a capacity building role with an understanding of funded and unfunded supports within the sector.

The successful candidate will be required to provide clear probity checks including a NDIS Worker Screening Check, Working with Children Check and valid Driver Licence prior to commencing employment.

The base salary for this role is between $74,000 and $79,000 (based upon experience and qualifications) plus superannuation.
