NDIS Support Facilitator

We are now seeking internal interest from those who are eager to diversify their skills and experience, taking on the role of NDIS Support Facilitator.

The Opportunity:

Based at our head office in Edney Lane, the Individualised Supports team are seeking to appoint a NDIS Support Facilitator within a fixed-term 12 month Parental Leave coverage position. The Support Facilitator is responsible for ensuring the provision of high-quality services that are person-centred, based on individual needs and choices of the participant and their family. 

About the role:

Upon joining our team, some of your day-to-day role responsibilities will include:

  • Ensuring service provision aligns with support items in NDIS plans and Service Agreements
  • Ensuring service provision is informed by participant needs and aligns with current protocols and individual service plans
  • Liaising with other providers for nominated participants to ensure consistency of supports in line with individual goals
  • Deliver positive support service options to people with a disability and their families
  • Roster and monitor Disability Support Workers to provide support to participants at their place of residence
  • Record, monitor and maintain participant and employee data.

About you:

To be successful in this role, you will bring with you:

  • Knowledge of, or experience in, the delivery of complex community services
  • Understanding and commitment to person-centred service delivery
  • Experience in planning and scheduling of activities, including rostering
  • A demonstrated understanding and the application of Work Health and Safety
  • Demonstrated ability to take initiative and work harmoniously in a busy team environment
  • Proficiency in using internal systems to complete daily tasks.

The part time rate for this role ranges from $37.35 - $40.05 per hour, to be determined based on skills, qualifications and experience. Working on Thursday and Friday within a 30.4 hour per fortnight contract, the successful candidate will commence on 24th July 2025, guided through a one week hand over.
