We are seeking a compassionate and organised NDIS Support Coordinator to join our dedicated team to make a real impact by delivering tailored support coordination services for individuals with complex needs. This role focuses on working alongside participants, their families and support networks to help them understand, implement and get the best outcomes from their NDIS plans.
About the role:
- Support participants to understand their NDIS plan and make informed choices.
- Undertake comprehensive assessment and planning for individuals with complex needs and facilitate referral to the most appropriate support services, aligned with their level of support needs and required intensity of care.
- Coordinate and link service users to appropriate care and community services across health, housing, disability, financial and related sectors.
- Apply a strong understanding of the community service sector and complex care issues to support holistic and person-centred outcomes.

- a base salary ranging from $40.81 per hour with competency-based increments up to $47.92 per hour, plus 12% super

We are seeking:
- A qualified professional with a minimum qualification of Diploma in Case Management (or similar relevant tertiary qualifications), and proven experience working with complex service users within community settings.
- Practical experience in one or more phases of case management as defined by the Case Management Society of Australia.
- Experience in disability, community services, social work, or a related field
- Strong communication, report writing, conflict resolution and negotiation skills, with a focus on delivering excellent client service and managing challenging situations.
- Strong knowledge of the NDIS and local service systems
- Proficiency in using personal computers and electronic assessment tools, with strong self-management and organisational abilities.
- Relevant clearances and certifications including: a current NSW Working With Children Check, NDIS Worker Screening Check (or willingness to obtain), current Criminal Record Check, valid Driver's Licence (Car) and a current Senior First Aid Certification.

This position is based in Tweed Heads South and can be directed to work from any Council work site.
