Your new role
Join the team dedicated to providing case management support to people we assist within the New England region who are experiencing substance abuse or dependence. Your goal as Case Worker in AOD Outreach Program is to develop holistic treatment plans targeted to individual needs of clients. Role will involve travel to locations within New England to deliver specific AOD services to smaller communities.
This role will report to the Team Leader, Health - Rosalie House and New England AOD Outreach. You can find more information about this role on the the attached Position Description.
The Case Worker – AOD Outreach Program will:
- Conduct assessments and develop evidence-based treatment plans and therapeutic interventions for individual clients
- Work in partnership with broader health services e.g. mental health services and other primary health clinicians, Aboriginal medical services
- Maintain accurate client records, case notes and other data as required
- Provide ad hoc support as needed, e.g. psychosocial support, conflict resolution etc
- Engage in case review processes with the team, and individually with the Team Leader.
To be successful in this role, you will need:
- Relevant qualification/s, e.g. Certificate IV or Diploma in Alcohol and Other Drugs or Community Services field, or approved counselling / welfare / psychology equivalent
- Demonstrated experience in managing a case load and case planning, including with persons with complex needs
- Demonstrated ability to work therapeutically with clients (e.g. provide therapeutic interventions).
- Knowledge of, and an ability to access, a range of relevant community resources.
