Your new role:

The Comorbidity Case Worker will provide specialist mental health support and coordination to eligible clients of Rosalie House or Freeman House . You will provide vital support to people who present with both substance use concerns, mental illness and other social or physical complexities. Your responsibilities: 

  • Provide psychosocial support to individuals with comorbidities within the North West Health Services operational program protocols
  • Deliver therapeutic interventions, support and education in accordance with the Society’s AOD Model of Care.
  • Conduct intake and comprehensive assessment (including risk assessment) of clients.
  • Support clients to develop holistic, person-centred multiagency care plans across a range of domains and stages of treatment.
  • Maintain accurate client records, case notes and other data as directed and required.
  • Be responsive to program needs and contractual obligations in line with organisations policies, procedures and contractual requirements.
  • Participate working parties. quality improvement and accreditation activities as directed

You will need: 

  • Relevant tertiary qualification/s in Social Work or Psychology
  • Life experience related to substance use and/or mental health. 
  • Demonstrated ability to establish effective workplace relationships with a diverse range of professionals and stakeholders.
  • Demonstrated experience in case management including; managing a case load, case planning and assisting clients with implementing case plans, including with persons who have complex support needs (e.g. co-occurring drug and alcohol issues along with mental health, homelessness or contact with criminal justice system)
  • Willingness to work within the established North West Health Services, the Society’s AOD Model of Care & the NSW Health AOD Clinical Care Standards.
  • Demonstrated ability to conduct self in a professional, ethical and non-judgmental manner.
  • Knowledge of, and an ability to access, a range of relevant community resources/service providers in local area. 
  • Basic computer literacy skills including experience in computerised data collection.
  • Well-developed time management and organisational skills.
  • Demonstrated high standard of interpersonal and communication skills, including the ability to adapt services to meet varying cultural needs.
  • NSW driver licence (unencumbered).
