About the role
Wintringham’s Housing and Homelessness Support (HHS) team is seeking an experienced Case Worker to join our team. The role provides housing support to Wintringham residents at Lyndell White House and Gilgunya. Reporting to the HHS Coordinator responsibilities include:
- Conduct initial and ongoing client assessments, develop support plans, and maintain accurate client records.
- Identify client needs, gaps and risks, and develop appropriate support strategies and responses.
- Link clients with clinical services, social and recreational activities, and higher-intensity supports such as HCP and NDIS.
- Support clients to maintain their health, wellbeing, tenancy, independence and connection to community.
- Uphold service quality, client rights, duty of care, privacy, policies, professional standards and continuous improvement
What are we looking for?
- A tertiary qualification in Social Work, Welfare, Community Services or a related discipline, or extensive experience working in the homelessness or community services sector. First Aid Level 2 and a current driver’s licence and reliable vehicle.
- Experience in support work and/or case management, with relevant training or experience in AOD, mental health, family violence or homelessness services.
- Strong understanding of the housing and homelessness sector, My Aged Care, NDIS and related service systems.
- Demonstrated commitment to best practice service delivery, accountability, innovation and continuous quality improvement.
- Well-developed communication skills, commitment to social justice, and ability to engage effectively with people from diverse backgrounds.
What we offer
- Employment type: Permanent full-time role
- Pay: $42.63 to $51.13 per hour plus super (depending on your qualifications & experience). Above Award pay and not-for-profit salary packaging benefits of up to $18,500 each year.
