About the role

Wintringham’s Tom Fitzgerald is seeking an experienced Care Manager to join our team. Lead the way in delivering exceptional resident care. As our Care Manager, you’ll be the key point of contact for care concerns, building a strong understanding of each resident’s individual needs and ensuring their care is person-centred, responsive and always of the highest standard.

Responsibilities include:

  • Assess and monitor the care needs of all residents
  • Assist with the development and implementation of detailed personal care plans, assessments and programs as required
  • Be available to support care staff to assess residents changing care needs
  • Liaise directly with the Clinical Care Team to discuss any care concerns
  • Refer to allied health/external services as required, for specialist assessments of individual resident needs
  • Manage changing care needs of residents as assessed and/or raised at the weekly care meeting
  • Respond to acute and/or emergency situations as the delegated person in charge and notify the Site Manager as required
  • Contribute to the evaluation, review and development of procedures
  • Coordinate ROD reviews, care plans, medication charts and AN-ACC.
  • Coordinate respite and general admissions into the RAC home.  This includes arranging admissions, finance, resident orientation and medication
  • Ensure staff have the information, instruction, training and supervision that they need to work safely

What are we looking for?

  • Division 2 Nursing Registration with Medication Endorsement
  • Knowledge and understanding of the special needs of older people
  • Experience working with clients with challenging behaviour
  • Experience in the assessment and management of changes in clinical needs of older people
  • Ability to work as part of a team
  • Good communication skills both written and oral.

 

What we offer

Employment type: Full Time, Fixed term Role (Parental Leave Cover) from 26/10/2026 to 30/04/2027

Pay: $57.55/hour – $62.66/hour (depending on your quals & experience) plus super. Above Award pay and not-for-profit salary packaging benefits of up to $18,500 each year.

Location: Tom Fitzgerald - Shepparton(VIC)

Roster/hours of work: 10 shifts per fortnight

Week 1: Monday to Friday (0700 – 1506)

Week 2: Monday to Friday (0700 – 1506)
