About the Organisation 

Sunrise Health Service Aboriginal Corporation (SHS) is a community controlled Health Organisation.

The SHS approach involves community people taking part in controlling their own health. Everything from financial management and governance, staff selection and service delivery priorities are directed by the organisation's Aboriginal Board and Community Health Committees.

SHS works in partnership with Northern Territory PHN (NT PHN), who provide support services to health professionals and organisation across the Northern Territory. NT PHN offers support and assistance to eligible nurses and allied health professionals who are relocating to the NT for the purposes of employment.

About the Opportunity

SHS has a unique opportunity for a Population Health Outreach Manager to join their dedicated team based in Katherine.

Reporting to the Director of Public Health and Planning, the successful applicant will be required to manage and support the Outreach team, ensuring the effective coordination of activities with both a clinical multi-disciplinary component and a health promotion approach. You will work closely with the Primary Health Managers and portfolio holders in all communities as well as engagement with community health committees. 

Some of your key responsibilities will include (but will not be limited to):

  • Facilitating regular case conferencing with allied health, primary health care staff and GP's; 
  • Implementing and improving processes through a continuous quality improvement focus;
  • Addressing relevant clinical system issues in partnership with the Clinical Governance Group;
  • Working closely with community engagement staff to ensure linkage of the Outreach Team through community consultations; and
  • Liaising with the Corporate Governance manager to ensure timely reports to funders and relevant proposals to other funding streams/programs in the area.

Although the SHS office is located in Katherine, you will need to be prepared to travel into the communities as required (not likely to be more than a few times per month). SHS will provide accommodation and travel allowances for any overnight stays while working away from home.

About You

SHS is seeking a highly dynamic individual to coordinate and facilitate the needs of the Outreach team. The ideal applicant will have a broad, experience based perspective that allows them to identify improvement opportunities within existing processes, and anticipate the nuanced requirements of managing multi-faceted operations. They will also have a confident, proactive attitude in order to affect improvements and nurture a friendly, positive working environment.   

It is important that the successful applicant exhibits an exemplary level of interpersonal communication skills, and is able to liaise comfortably with people at all levels. 

Demonstrated management experience leading a multi-disciplinary team will be essential, and experience working in remote locations will be very highly regarded. The successful applicant will also ideally have a background in an AHPRA registered health profession, or equivalent experience.  

The successful candidate must be willing to undergo a Working With Children Check or hold a current Ochre Card and have an unrestricted NT drivers licence.
