ABOUT THE ROLE As a Care Facilitator, you will empower clients to live independently with dignity and respect. You’ll: ·Plan and coordinate person-centred care services ·Build strong relationships with clients, carers, and service providers. ·Manage care plans, budgets, and service agreements (NDIS, CHSP, HCP). ·Mentor new staff and contribute to a positive team environment. ·Ensure compliance with quality standards and Council policies. ·Use your initiative and professional judgement to solve challenges and deliver great outcomes.
WHAT WE’RE LOOKING FOR
Essential:
·Knowledge of Aged Care Quality Standards & NDIS Practice Standards
·Excellent communication and documentation skills
·Strong computer skills (Microsoft 365 apps)
·Class C driver’s licence
·Ability to meet all relevant screening and police checks
Desirable:
·Certificate IV in Community Services or related field
·Experience in budgeting, accounting, or social work
·First Aid Certificate
·Experience with client management database systems
