ABOUT THE ROLE   
As a Care Facilitator, you will empower clients to live independently with dignity and respect.  You’ll:
·         Plan and coordinate person-centred care services
·         Build strong relationships with clients, carers, and service providers.
·         Manage care plans, budgets, and service agreements (NDIS, CHSP, HCP).
·         Mentor new staff and contribute to a positive team environment.
·         Ensure compliance with quality standards and Council policies.
·         Use your initiative and professional judgement to solve challenges and deliver great outcomes.

 

WHAT WE’RE LOOKING FOR
Essential:
·         Knowledge of Aged Care Quality Standards & NDIS Practice Standards
·         Excellent communication and documentation skills
·         Strong computer skills (Microsoft 365 apps)
·         Class C driver’s licence
·         Ability to meet all relevant screening and police checks
 
Desirable:
·         Certificate IV in Community Services or related field
·         Experience in budgeting, accounting, or social work
·         First Aid Certificate
·         Experience with client management database systems
