We are looking for a Clinical Nurse Comprehensive Assessment and Documentation (CN CAD) with a real passion for caring for our elderly (or for aged care), driven to achieve high quality assessments, care plans and resident outcomes to join the CAD Team in the Adelaide Metropolitan Region on a permanent full time basis. Reporting to the Manager Residential Funding, the CN CAD will be based at a primary metro location but may also work at other Helping Hand Residential care facilities as required.
This position is an integral part of the broader CAD Team who work closely with site Residential Services Managers, senior clinical staff and allied health practitioners, to complete accurate assessments and facilitate timely development and review of person centred care plans. The CN CAD, in collaboration with the broader CAD Team, is responsible for the comprehensive contemporary assessment and care planning for all residents, enabling each resident’s quality of life and independence to be restored and maintained at an optimum level. The CN CAD completes all applications of the Aged Care Funding Instrument (ACFI) to ensure subsidy income is maximised.
This position is a fixed term full time position- paternity and annual leave cover for 6 months commencing on 1st Feb- 2nd August 2021.
Skills & Experience
To be considered for this role you must possess a current registration and practicing certificate with Australian Health Practitioner Regulation Agency (AHPRA), as a Registered Nurse or Allied Health Professional. You will also be able to demonstrate:
/ Ideally, ACFI experience of at least 2 years
/ Demonstrate experience in and sound knowledge of the application of comprehensive contemporary assessment and care planning
/ Demonstrate experience in a leadership role including workload management, project management and critical and reflective thinking skills in decision making and problem solving
/ Communicate with a range of people including older people, staff from all disciplines and levels, volunteers and visitors, particularly in relation to care of residents
/ Understand the principles and contemporary practice relating to change management
/ Demonstrate high understanding and experience in working within regulatory and legislative practice parameters in relation to nursing practice
/ Good written and verbal communication skills including experience in preparing reports, analysing data and providing recommendations
/ Demonstrate commitment to continuing professional and personal development.
