About the Program
The Bilingual Hospital to Home (H2H) program supports people living with mental ill health and people who have self-harmed or attempted suicide; for up to 12 weeks following discharge from hospital. Bilingual H2H is a psychosocial model and activity includes referrals to community supports to build capacity and work towards recovery and resilience.
We help people to connect, to live well in their community.
About the Role
Working in this role, you will support the team, external stakeholders and individuals with psychosocial disability to assist program workflow and a positive intake experience for referrers. This role has been designed to triage and manage all Bilingual H2H program enquiries from various referral points, contributing to the programs role in a stepped care model of mental health support.

Your skills and experience
- Relevant qualifications (Diploma or above) in mental health, health or welfare degree, or Cert IV in Mental Health/Peer Work or equivalent level of expertise attained through previous appointments, service and study
- An understanding of the recovery- based approach to assisting people with severe and persistent mental illness
- Knowledge of the NSW mental health systems and local services
- Fluency in CALD language/s and English. Speakers of Farsi/Dari, Arabic, Hindi, and Mandarin and other community languages of the region are encouraged to apply
- Ability to communicate effectively in all interactions with all stakeholders

Additional Details
One Door Mental Health requires all staff to have a Police Check clearance, a current Working with Children's Check and NDIS Worker Screening clearance and appropriate COVID-19 Vaccination.
